In this post we discuss team formation.

Process Hazards Analyses (PHAs) are generally conducted by a team (although some of the more specialized techniques, such as Fault Tree Analysis, may be conducted by a specialist working alone or with just one other person). The team members should represent a cross-section of disciplines and functions at the facility or on the project — at a minimum including operations, engineering, maintenance, and process design.
