BSEE (the United States Bureau of Safety and Environmental Enforcement) is the regulator for offshore oil and gas operations. They have just issued Safety Alert No. 485 ― Improper Use of Hand Tools Poses Injury Risk. It describes two incidents in which workers suffered hand injuries while using a handheld drill.

Although these events are more to do with occupational safety rather than process safety, they do demonstrate the importance of personal safety procedures and precautions..
